PLANNED ABSENCES

If you plan for your child to be absent from school, please fill out this form one week prior to the absence. Deliver it to the school office in person or by fax (587-2092) to insure that teachers have time to prepare assignments and schedule makeup test.

IMPORTANT - MEDICATION ALERT

Oakbrook Schools can only dispense prescription medications and over the counter drugs recommended by your doctor. These medications must be submitted to the school office along with the medication form signed by the doctor. No medication can be dispensed without this documentation. Please send all medication to school in labeled containers, which include name of child, name of medication and dosage requirements. No student may have any prescription or non-prescription medication with him/her at any time. Medication will be administered by the nurse only.

2007-08 SCHEDULE OF REQUIRED VACCINATIONS, SCREENINGS, AND IMMUNIZATIONS FOR SCHOOL ADMITTANCE

Pursuant to Regulation 61-8, the South Carolina Department of Health and Environmental Control has declared the following schedule of required vaccinations, screenings, and immunizations necessary for a child to be admitted to any public, private, or parochial school, grades kindergarten through twelve (K-12), or child development program under the control of the State Department of Education for the 2007-08 school year.

Minimum requirements:

The S.C. Department of Health and Environmental Control has declared the following schedule of required immunizations for the 2007-2008 school year:

* Four (4) doses of any combination of DTP, DT, DTP-Hib, DTaP, or Td vaccine with at least one (1) dose received on or after the fourth birthday. (This immunization requirement only applies to students entering five-year old kindergarten, first, second and third grades in school year 2007-2008. Students previously enrolled in grades 4 through 12 met the requirement as stated for the school year in which they entered school.)

* Three (3) doses of any combination of oral or inactivated Polio vaccine with at least one (1) dose received on or after the fourth birthday.

* Two (2) doses of Rubeola (Measles) vaccine with both doses received on or after the first birthday and separated by at least one month.

* One (1) dose of Rubella (German Measles) vaccine received on or after the first birthday.

* One (1) dose of Mumps vaccine received on or after the first birthday.

* Three (3) doses of Hepatitis B vaccine.

* One (1) dose of Varicella vaccine received on or after the first birthday or positive history of disease for all children admitted to kindergarten, first, second, third, fourth, fifth, sixth and seventh grades.

A group of Moms who meet to pray for our school and teachers meet every Wednesday 8:30 am.

SAFETY ALERTS

The lower school parking lot is for teachers, staff, and high school students only.
NO student should be dropped off or picked up from lower school parking lot between the hours of 7:30-3:00 pm.

PLEASE USE THE CROSSWALK AT ALL TIMES.

 

SCHOOL HOURS

Any student who arrives after their stated arrival time must report to the front desk for a late slip. A student who comes to school late, without a legitimate excuse ( health care appointment), will be marked as an unexcused tardy.

*Remember to pick up at the time your oldest child is dismissed.

CARPOOL INFO

> Park ONLY in designated parking spaces or on the front lawn.
> Do not block the drives to the Primary Building.
> Do not block the crosswalk.
> Pick up ALL children at the dismissal time of the OLDEST child.
> If picking up children at Primary and Main Buildings, pick up at the Primary Building first and then merge into the main carpool loop for pick-up at Main Building.
> DO NOT drop off or pick up in parking lot of Gym Building.
